Policies

ALL CLIENTS

A credit card on file is required for cash pay and TLN covered appointments and is due in order to book your appointment.
 

Payments may be made electronically using a credit card or funds transfer. Jen Kowal IBCLC and Milkwell use SquareUp to process payments. SquareUp meets the high standards of HIPAA and the banking industry for security and privacy with regard to financial transactions. However, SquareUp may send, automatically or per your request, email or text message receipts that reveal personal health information such as the date and type of lactation visit. You do have the right to refuse payment via SquareUp at the time of booking and instead pay via automatic bank fund transfer, cash or check. If you decline online payments, you will need to reach out so your appointment can be booked manually and payment arrangements can be made. This will result in delays. Credit card on file is still required per practice policies.

Insurance

 

Aetna + Cigna: We are members of Lactation Consultants of America, a nationwide group practice of in-network providers. As such, Aetna + Cigna clients are clients of LCA with services performed by Milkwell.  Clients will have financial agreements directly with LCA only and will receive intake forms etc from LCA.   LCA handles all billing and pays Milkwell as a contractor. Clients will not receive additional bills for 1:1 services from Milkwell but should expect to receive bills from LCA for the patient responsibility of each claim.  The LCA Payment Policies will outline financial obligations and expectations. Please direct all billing related questions to benefits@lcsofamerica.com.

 

United, BCBS, + Anthem: We collaborate with The Lactation Network to bring insurance covered lactation care to the families who wish to work with us but who would have to submit for their own reimbursement otherwise.  Your lactation care is performed by Milkwell and TLN submits claims to insurance carrier and pays the lactation professional for the lactation services.  These clients will have financial agreements with both TLN and Milkwell. According to TLN's Policies, clients should not expect to receive bills from TLN for any patient responsibility however Milkwell is permitted to charge an administration fee directly to clients on top of the payment from TLN.  This fee is for non-clinical, non-billable services and is in no way meant to be construed as balance billing.  Please direct TLN billing related questions to care@tln.com and questions regarding Milkwell fees to hello@milkwelllactation.com.

SELF-PAY

Please note that reimbursement for self-pay clients is not guaranteed. You are fully responsible for the out of pocket fee up to the amount of the cash pay fee regardless of how much insurance ends up reimbursing you. If you are concerned about reimbursement, please call your insurance carrier for more information.

Full payment is required for lactation consultations and is due in order to book your appointment unless alternative arrangements have been made in advance, for instance, you are experiencing financial hardship and you have been in touch with the office about making alternative payment arrangements. Cancellations of less than 24 hours will result in a charge of $75.  Please provide more than 24 hours notice when cancelling or rescheduling your visit to avoid the fee.  Any refunds that may be issued will be minus the credit card service charge that we are charged by the credit card  processing company.

Clients are responsible for all charges associated with any past and/or future visits with Milkwell. Milkwell will charge credit cards on file for outstanding balances. Milkwell will attempt to contact clients prior to processing said payment via phone and email.  If client fails to pay the outstanding invoice(s), within 24 hours of the visit, Milkwell reserves the right to process the payment via stored credit card.

You will be provided with a superbill suitable for you to submit to your insurance. The superbill (which will also serve as a payment receipt) will be coded appropriately to the level of service provided during the visit. You agree to pay Jen Kowal IBCLC  prior to or at the time of the visit (cash, check, credit card, or FSA). 

Milkwell does not provide refunds.  You may reschedule any cancelled appointment for a future date up to 90 days from the originally scheduled date.

In order to be reimbursed by insurance, some policies require pre-authorization. Call your insurance carrier and request an "in-for-out" meaning in-network coverage for an out-of-network provider, and/or a Gap exception; ask the insurance company to cover the services as in-network to fill the "gap" they have in their own network by not actually having any in-network lactation consultants.   Request your insurance cover numerous visits because often we need to follow up to continue to improve feeding and comfort. Please see our blog post about GAP EXCEPTIONS. All of the information you will need to complete this documentation is provided in this blog.  Our office is not able to complete these forms for our clients.

+UrgentCare

These 30 minute virtual visits allow you to have the support you need without having go through the lengthy registration process or waiting for benefits verification. Once we receive your request we will be in touch to set a time within 24hours of the request.

Your credit card will be captured during the booking process and will NOT be kept on file.  In the event of cancellation of the +UrgentCare request, the fee is non-refundable. No superbill will be provided, services not eligible for insurance reimbursement.  If you need your services covered by your insurance benefits, please choose another appointment type.  If you book this visit by mistake there will be no refund but the fee can be applied to a future service, class, group or workshop as you choose.
